Configurable safety forms
Configurable safety forms puts your own safety paperwork on your crews' phones. Your forms are set up during onboarding, and every filed record is sealed and numbered.
Request a demoYour paperwork, on a phone
A form can be a JHA, a near miss, an incident, an inspection, a permit or a training record. Questions can take written answers, numbers, dates, choices, checkboxes, photos, signatures, and hazards with their controls. Signatures are drawn on the phone, and a form with no signature block asks for the person's name. Photos are switched on per form, two to a question by default and up to six.
At the site, with no login
Crews open the site's forms page on their own phones and type the site's code once. The phone then stays set up for 180 days, and a printable QR poster opens the page. Answers are saved on the phone as they are typed, even when the screen locks. With no signal, the screen says NOT SUBMITTED and offers Try again, and a retry cannot file the record twice.
Checked before it is filed
Required questions are checked on the phone and again on the server. A stop rule on a form refuses an answer that breaches its limit, and nothing is saved. A check flags records that need a look, such as a JHA filled in under 35 seconds.
Sealed, numbered and kept
Each record gets a document number for its kind and year, such as PERMIT-2026-00001. Every record is sealed into a hash chain, and its page reads Unchanged since it was filed while the seal matches. A correction is a new revision with the same number and a written reason, and the original stays readable. Each record downloads as a PDF.
Permits, close-outs and KPIs
The Permits page shows whether each permit form has Stop Work Authority armed or is advisory. The KPIs page covers 7, 30 or 90 days, for the whole account or one project, with no names on it. TRIR reads Not calculable when no hours are recorded.
What Configurable safety forms is not
There is no form builder in the app, and your forms are set up during onboarding. It does not send a form later in the background when signal returns. Crews do not correct their own filed forms. Filing a form does not send a push notification.

- Not in the app. Your forms are set up for you during onboarding.
- Do crews need a Panora login?
- No. They type the site's code once on their phone, and the phone stays set up for 180 days.
- What happens when there is no signal?
- The record is not sent until someone taps Try again with signal. The answers stay saved on the phone meanwhile.
- Who can see filed records?
- Admins and safety managers. Viewers cannot open the Safety forms, Permits or KPIs pages.
