Safety findings

Safety findings turns a report filed on a phone into work with an owner, a due date and a record of what was done. Each finding moves through four stages, from Not Started to Complete.

The findings loop for a sample site: six counts for open, overdue, due in 7 days, closed and verified in 30 days, and median days to close, then filters and a board with Not Started, Assigned, In Process and Complete columns.

Report it from a phone

Each facility has a QR code that opens the safety finding form on a phone. The form takes a description, the type of submission and concern, a priority, photos and a signature. If reporting is not ready at a facility, its card says why. The filed report itself is never changed.

An owner, a due date, a dated log

Assign a safety committee member, a user, or someone else by name. Set a due date, and move the finding to In Process when work starts. Each log entry is added with the writer's name and the date, and earlier entries stay as written.

Closed with a note, reopened with history

Closing a finding needs a verification note that says how the work was confirmed. Reopen it when more work is needed, and earlier changes stay in its history. Every change is saved with who made it, when, and the value before and after.

The committee dashboard

Charts show open work per owner, open findings by stage, and counts by type of submission and type of concern. A strip shows how long open findings have waited: 0 to 7 days, 8 to 14, 15 to 30, and over 30. This year's closed count says whether it is up or down from last year at the same date. Each facility lists its safety committee, and admins and safety managers can edit it.

The findings loop

With the loop on, a Raise a finding button sits on any filed record, not just the finding form. Closing asks for a verifier, and a different named person must verify the close-out. A new finding in the same category at the same site within six months, by default, is marked as a repeat. Patterns show findings per month, time to close, and the categories that keep coming back.

What Safety findings is not

It never edits the report a worker filed. It does not close a finding without a verification note. Notices are email alone, and they stay off until an admin turns them on.

Who can change a finding?
Admins and safety managers assign, update and close findings.
Does it send notices?
With the findings loop on, an admin can turn on email notices. They cover assignments, due dates, a daily overdue digest, close-outs and verification.
Does a finding change the report it came from?
No. The filed report stays as it was signed, and the work on the finding is kept in its own record.
Can we take the list out of Panora?
With the findings loop on, the filtered board downloads as a CSV file.
